Patients Information

University Pediatric Association requests that all new patients arrive at least 30 minutes prior to appointment time so that we can adequately process paperwork. Please have your current insurance card and your driver's license, so that a copy can be made for your child's file.

We also require that the parent or guardian sign a copy of our financial policy the first time you are seen by our doctors. This document will fully explain our financial policies.
